Privacy Practices

Effective Date: September 2, 2026

Oasis Counseling Center LLC, doing business as Oasis Behavioral Health (“Oasis,” “we,” “our,” or “us”), is committed to protecting the privacy and confidentiality of the personal and health information entrusted to us.

This policy explains how we collect and protect information through our website, how protected health information may be used and disclosed, and the rights clients and patients have concerning their clinical records.

Information Collected Through Our Website

When you visit our website or contact us electronically, we may collect:

  • Your name, telephone number, email address, and other contact information.
  • Information submitted through contact, appointment-request, insurance-verification, or inquiry forms.
  • Technical information such as your IP address, browser, device type, referring website, pages visited, and general website-use information.
  • Information collected through cookies or similar technologies used for security, basic functionality, preferences, and website performance.

We use this information to respond to inquiries, coordinate appointments, provide requested information, operate and secure our website, improve our services, and comply with applicable legal obligations.

Oasis does not sell protected health information.

We may use qualified service providers to support website hosting, communications, scheduling, billing, security, or other operations. When a service provider creates, receives, maintains, or transmits protected health information on behalf of Oasis, we require appropriate privacy and security protections, including a Business Associate Agreement when required by HIPAA.

Website Forms and Electronic Communications

Please provide only the information reasonably necessary for us to respond to your inquiry. Website forms and ordinary email may not be appropriate for detailed clinical information.

Electronic communications may carry privacy risks. You may request that Oasis communicate with you through a particular telephone number, email address, mailing address, or other reasonable method.

This website is not monitored as an emergency or crisis service. If you are experiencing an emergency, call 911 or go to the nearest emergency department. For suicide or crisis support in the United States, call or text 988.

Our Responsibilities for Protected Health Information

Oasis is required by law to:

  • Protect the privacy and security of protected health information.
  • Provide individuals with notice of our legal duties and privacy practices.
  • Follow the privacy notice currently in effect.
  • Limit uses and disclosures of protected health information as required by law.
  • Notify affected individuals following a breach of unsecured protected health information when notification is required.

We may revise this policy when our practices or legal obligations change. The current version will be posted on this website with its effective date.

How We May Use and Disclose Health Information

Treatment

We may use and disclose health information to provide, coordinate, and manage your care. For example, members of your treatment team may share information necessary to complete an assessment, establish a diagnosis, develop a treatment plan, coordinate medication management, or make an appropriate referral.

Payment

We may use and disclose health information to verify insurance benefits, obtain authorization, submit claims, collect payment, respond to a health plan, and perform other lawful billing activities.

Healthcare Operations

We may use and disclose health information for appropriate business and clinical operations, including quality improvement, care coordination, staff supervision and training, credentialing, auditing, compliance, legal services, and business administration.

Other Uses Permitted or Required by Law

Subject to applicable legal requirements, we may use or disclose information:

  • When required by federal or state law.
  • To report suspected abuse, neglect, exploitation, or certain threats to safety.
  • For lawful public-health or health-oversight activities.
  • In response to a valid court order, subpoena, or other lawful process.
  • For workers’ compensation or similar programs.
  • To prevent or reduce a serious and imminent threat to a person or the public.
  • To a personal representative or another person legally authorized to act for you.

We will limit disclosures to the information permitted or required by law.

Uses Requiring Written Permission

Oasis will obtain written authorization or consent when required by law, including for many disclosures that are unrelated to treatment, payment, or healthcare operations.

Written authorization is generally required for most uses of psychotherapy notes, most marketing activities involving protected health information, and the sale of protected health information.

You may revoke an authorization or consent in writing, except to the extent that Oasis has already acted in reliance on it or another legal limitation applies.

Substance Use Disorder Records

Records identifying a person as having or having had a substance use disorder may receive additional protection under 42 C.F.R. Part 2.

When these protections apply:

  • Oasis will use or disclose substance use disorder records only as permitted by federal law.
  • Written patient consent will be obtained when required.
  • A patient may revoke consent as permitted by law.
  • Uses and disclosures not described in this policy will be made only with the patient’s written consent unless another legal exception applies.
  • Substance use disorder records generally cannot be used against a patient in a civil, criminal, administrative, or legislative proceeding unless specifically authorized by the patient or permitted by a qualifying court order and accompanying legal mandate.
  • Records disclosed with consent for treatment, payment, or healthcare operations may be redisclosed as permitted by HIPAA when federal law allows.
  • Oasis will not use substance use disorder records for fundraising without providing any notice and opportunity to opt out required by law.

Your Privacy and Medical-Record Rights

Subject to applicable law, you have the right to:

  • Inspect or obtain a paper or electronic copy of your clinical and billing records.
  • Request a correction or amendment if you believe information in your record is inaccurate or incomplete.
  • Request that we communicate with you through a particular reasonable method or at a particular location.
  • Request restrictions on certain uses or disclosures.
  • Require us not to disclose information to a health plan about a service for which you paid Oasis in full out of pocket when the legal requirements for that restriction are met.
  • Request an accounting of certain disclosures of your information.
  • Receive a paper or electronic copy of this policy.
  • Authorize another person to act as your legally recognized personal representative.
  • Discuss this policy and your privacy rights with the Oasis Privacy Officer.

Some requests must be made in writing. Oasis may deny or limit a request when permitted by law. If that occurs, we will provide an explanation and information about any available review rights.

A reasonable, cost-based fee may apply to certain record-copy requests when permitted by law.

Our website may contain links to third-party websites. Oasis does not control the privacy, security, or content practices of those websites. We encourage visitors to review the privacy policy of any third-party website they use.
